Maury Regional Health offers several ways to obtain your medical records, depending on the type of information you need and preferred format.

• Print and complete an authorization form (links below) and provide it to the appropriate Maury Regional Health facility listed below.

To request copies of your medical records, please call the appropriate number below. A member of the medical records staff will verify your identity and work with you to determine where to send your medical records. If you need to pick up your records in person, please call in advance to make arrangements.

Urgent Requests from your Physician

For continuity of care, your health care provider can request records. The physician office should fax a request on their letterhead to 931.380.4092.
